Dog Licensing

ATTENTION DOG OWNERS!

State law requires all dogs to be licensed and vaccinated for rabies. Proof of vaccination from the veterinary clinic must be presented including the expiration date and the vaccination manufacturing serial number at the time of licensing. A dog license will not be issued without proof of vaccination.

All dogs must be licensed by April 1 each year. Dogs licensed after the due date of April 1 will be assessed a late fee of $5.00 per license.

To obtain a license for your dog -
Complete the dog information form. Send the completed form, proof of current rabies vaccination, a self-addressed stamped envelope, and a check payable to the Village of Mt Calvary to:
